About Your Provider

Our provider, Dr. Jolene Thweatt, is a highly skilled and experienced family nurse practitioner who is dedicated to providing the best possible care to our patients.


ABOUT DR. JOLENE............  


"I am a very proud mother of 4 boys who are my life. My husband, a journeyman lineman has been the support for our family and my practice. Our children Cole, 27 and Preston, 25, are both in the line trade for IBEW Local 769. Waylon 17 is a wrestler and loves the UFC while Daniel, 16 attends the Electrical and Power Transmission class at EVIT and is Waylon's sparring bud. . 


We believe in Jesus Christ, our 2nd Amendment, the nuclear family, and live by conservative values. The American flag is on a lighted pole in the front yard 365 days a year and we support our military and our 45th and 47th President. 


I have 4 sisters and an Army Veteran brother who all live between West Virginia, Mississippi, and Ohio. . 


I love being a nurse practitioner in primary care and I love being able to do it the "right" way. I truly believe that medicine is as basic as the right for us to be healthy and protected by nature.

CONCIERGE MEDICINE SERVICES

$50/month per person

$100/month for a family of 5

Anyone over 21 years old does not qualify as a child and must have their own account


  

Primary Care Services Included 

New Patient Initial Visit ($120 value)

Patient Follow-up ($60 value)

TB Skin Test with Interpretation ($15 value)

Toradol Shot ($25 value)

Vitamin B12 Shot ($20 value)

Rocephin Antibiotic Shot ($30 value)

Strep Test ($10 value)

Pregnancy Test ($5 value)

Urinalysis ($10 value)

Breathing Treatments ($30 value)


 Weight Loss Services.

Weight loss (semaglutide and tirzepatide)- $60 for initial visit with $30 per follow up visit mandatory every 3 months. 


Other Services ( Additional charge applies)

B12 10mL bottle with #10 needle kit- $60 (this includes $10 charge for shipping)

B12 30mL bottle with #30 needle kit- $120 (this includes $10 charge for shipping)

IV Hydration- $120/Liter, infused over 45 minutes (in-office)
FMLA paperwork- $100 per submission

Worker’s Compensation paperwork- $100 per submission

Sports Physicals- $25

CDL Medical Examination- $50 


We do not offer any vaccines or gender altering hormone replacement.
