At SFM, we know how important it is for you to maintain your health and have the best quality of life. We also realize that each person is unique and may have health concerns that are not being listened to by other providers. Our provider at SFM is prepared to offer you with the best options to meet your health challenges. She will provide you the opportunity to make an informed decision about your health with all options discussed at your appointment. We provide comprehensive primary care for children and adults of all ages, and assist with understanding and managing diseases. Some of the most common issues we treat are high blood pressure, high cholesterol, diabetes, asthma and COPD, skin irritation and rashes, hormonal imbalances, and infections. We also provide services for more urgent matters such as breathing treatments, suturing, and joint immobilization.
We also pride ourselves in providing DOT physicals for CDL drivers at the lowest rate you will find in Arizona. The entire exam and paperwork takes no longer than 20 minutes, and we email your certificate to motor vehicles with your email attached for receipt confirmation- There is no need for you to go to DMV and drop off your certificate. We also provide a laminated and foldable mini certificate for your wallet. Our practitioner is the wife and mother of IBEW powerline wokers and knows how important it is to maintain a commercial driver's license.

What Does the DOT Physical Exam Consist Of?
- Health history
- Focused Assessment/Exam
- Vision test
- Hearing "whisper" test
- Urine test for proteins, blood, and sugar levels
- NOTE! Testicular, prostate, and hernia exams are determined by your health history and personal concerns. These exams are not standard tests during your physical.
NOTE: Drug tests are NOT collected by the DOT medical examiner. This test will not be completed in this office and should be ordered to take place elsewhere by your employer.

We are a primary care practice that specializes in family medicine. Unlike other primary care practices, SFM has chosen to advocate for freedom and choice. We also do not have a cookie cutter so your visit is unique. We understand that not everything can be "fixed" with pills and that natural approaches can be extremely beneficial. We also know that not everyone has the money to treat their bodies as well as those in Hollywood so we get creative and use every resource available to us.
Sometimes all we need is someone to listen to us, help us brainstorm remedies, be a cheerleader and support system for us, and treat us like family. Going to see your primary care provider should not be a dreaded event and should ALWAYS be the first line of defense against illness and disease.
If your doctor is leading you into the "right" direction of your healthcare journey, you should need to see your doctor very little as time passes. Remember, this is a team effort, and your health outcomes are also determined about the amount of effort you apply. Nonetheless, you can be assured that we will do our part to ensure you are able to reach your goals!

For those who are coming to us for their DOT physicals, you will NOT encounter a better experience anywhere else and our price will NOT be beat!! All medical forms are electronically submitted to the Federal Motor Carrier Safety Administration (FMCSA) and Motor Vehicles Division, free of charge. We make certain the Department of Transportation has received your medical certificate, keeping your CDL license in good standing! You leave your visit with a copy of the exam certification card in hand and laminated!
